Getting straight to the heart of the attack
Heart problems are growing by the day is a given, but the low awareness about symptoms of a heart attack, and what instant action should be taken, are often key causes that aggravate the problem. Experts say about 40-50% patients remain unaware that they are experiencing a heart attack, which thereby leads to delayed visit to the hospital and availing treatment.
Cardiologists in the Bangalore have joined hands to start a campaign to increase awareness levels not just amongst patients, but the public in general, by introducing a booklet on understanding symptoms of heart attack, what should be done, risk factors, treatment options and case studies. Dr Vivek Jawali, member of the Journal of Indian Association of Cardiothoracic Surgeons, says the first hour post the heart attack is the golden hour. 'Treatment started within the golden hour is key towards restoring health of the patient. Facilities are available in each neighbourhood in Bangalore. But people should start brushing up their knowledge on symptoms of an attack and what should be done immediately.' Often many people having a heart attack wait for more than three to four hours before getting help, say doctors.
'Early recognition, awareness of heart attack and reaching hospital on time are key factors responsible for surviving a heart attack. Even when the situation seems completely out of control, expert medical care at the right time can turn things around,' says Dr Venketesh S, interventional cardiologist, Fortis Hospital.

At the core of the problem
India has 45 million patients suffering from heart ailments. The country sees 28.6 lakh heart attack incidents every year.
Almost 50% of deaths following a heart attack are due to dearth of immediate care within first hour of a cardiac emergency.
About 25% of deaths in the 25-69 years age group are attributed to heart diseases by doctors.
Symptoms of the onset
If a person is experiencing either pain, discomfort or heaviness in either the chest, neck, jaw, back, abdomen, arms or shoulders for 20 minutes or more, it could be a heart attack. Patients can also experience breathlessness or nausea, sweating and numbness.
Chest pain is not always a sign of heart attack. A patient could instead be experiencing sweating, vomiting, breathlessness, etc.
First aid in case of an attack
Dr Venketesh S, interventional cardiologist, Fortis Hospital, says as a first aid measure, aspirin (325 mg) that is easily soluble or chewable should be given to the patient. Reach out for a cardiac ambulance so that treatment can be started within the ambulance. And then reach out to the nearest hospital having facilities like ICU and ECG. Relatives of patients should educate themselves on the necessary forms of treatment to avoid delays in decision making.
Dr Vijay Surase, interventional cardiologist at Jupiter Hospital, Thane tells us about some classic symptoms of a heart attack that we should watch out for:
Common signs of a heart attack:
Discomfort or pain arising at the centre of chest.
Feeling of suffocation with chest pain
Difficulty in breathing
Some women and people with diabetes do not have classic symptoms. Women usually have no severe chest pain, but a general weakness and discomfort. This leads to women ignoring the problem until late. Mostdiabetics develop atypical symptoms like profuse sweating or loss of consciousness during a heart attack which may be misinterpreted as symptoms of low sugar in the body. Sometimes heart attack is diagnosed during investigations like ECG or echocardiography even though you may not have had any history of chest pain. Such silent heart problems are a major challenge because they progress rapidly towards heart failure.
Other signs and symptoms of a heart attack include:
Anxiety
Cough
Fainting
Light-headedness, dizziness
Nausea or vomiting
Palpitations (feeling like your heart is beating too fast or irregularly)
Shortness of breath
Sweating, which may be very heavy
Women too have to be on the watch out because they don't experience the 'classic' symptoms of a heart attack. Some common symptoms in women are:
Uncomfortable pressure, squeezing, fullness or pain in the centre of your chest. These pains are infrequent and come and go.
Pain or discomfort in one or both arms, the back, neck, jaw or stomach.
Shortness of breath with or without chest discomfort.
Other signs such as breaking out in a cold sweat, nausea or light-headedness.
As with men, women's most common heart attack symptom is chest pain or discomfort. But women are somewhat more likely than men to experience some of the other common symptoms, particularly shortness of breath, nausea/vomiting and back or jaw pain.

ENGLAND'S ALL-ROUNDERS
Wanted: one international all-rounder. Should be able to bat No6 and bowl fast. Ability to reverse swing the ball is ideal, experience in the first division of the County Championship is essential. All applications considered, some more seriously than others. England are considering the candidates. Before this one-day series started, their bowling coach David Saker said that "we are searching for players who can play that multi-task role" and that the "more all-rounders we have in the group, that's great."
This is a switch. Since Andrew Flintoff retired in 2009, England have been content to play three quicks and a spinner, supplemented by a second if conditions suit, and bolstered by a little help from their batsmen. Their predilection for picking and sticking with a four-man attack has been vindicated by their results in that time. It was a formula that won them three consecutive Ashes series, and made them the World's No1 team. Now though, they seem keen to bring in an all-rounder, mindful, perhaps, that this is one area in which South Africa, who have replaced England at the top of the rankings, have a clear advantage.
Ali Bacher, the old South African Test batsman and team manager has just written a history of his country's all-rounders, as rich a seam as almost any other in sport. It covers 13 of the finest, from Jimmy Sinclair through to Jacques Kallis. He lays claim to a couple of England's better ones too, Basil D'Oliveira and Tony Greig. Since those two finished playing, England, two conspicuous exceptions aside, seem to have been stuck with a succession of wannabes, never-will-bes, and never-weres, picking a succession of own-brand Ian Bothams, as Barney Ronay memorably called them, who never quite cut it in the 1990s.
Andy Flower has had an understandable aversion to playing an all-rounder just for the sake of balancing the side. He has been fortunate in that he hasn't had to. He has been blessed with a strong lower-order of Matt Prior, Stuart Broad, Graeme Swann, and, when he's been picked, Tim Bresnan. Since Andrew Flintoff retired there haven't been many – any – all-rounders Flower felt worth accommodating.
Warwickshire supporters will feel that was a little unfair on Rikki Clarke. He was the last seam-bowling all-rounder to make his Test debut for England, against Bangladesh back in 2003. Until, that is, they picked Chris Woakes to play at The Oval. He's now back at Warwickshire, and Ben Stokes is being given a run in the one-day team. The two of them are on trial. There are a few others around too. England's current squad also includes Chris Jordan, who has blossomed as a fast bowler since he quit Surrey for Sussex, taking 52 wickets at 27 this season, but whose natural talent with the bat remains relatively unfulfilled. Clarke himself, and Somerset's Craig Overton, both went on tour with the England Lions to Australia last winter.
It's telling though that England's selectors have spurned the two leg spinners, Adil Rashid and Scott Borthwick, who have both been batting brilliantly in the top six this season. Samit Patel is outcast too, to his obvious irritation. England seem very clear about the type of all-rounder they are looking for.
Both Stokes and Woakes have impressive records and good references. Woakes has played 84 first class matches, has a batting average of 37, with six centuries, and takes his wickets at under 26 apiece. When he was Warwickshire coach, Ashley Giles said that if he could clone one of his players, it would be Woakes. "Everything about him is impressive. He trains hard, he works hard on his fitness, he's receptive to new ideas and he's low maintenance," Giles said, "He has a future as a genuine all-rounder."
Stokes, on the other hand, has played slightly fewer first class matches, with 56, has a batting average of 36, with eight centuries, and a bowling average of 27. 'He is good enough to become a specialist No 6 batsman, with all that entails," Paul Collingwood said at the start of this season. "He bowls at really good pace and moves the ball both ways with conventional swing. And standing at slip to him against Warwickshire, I saw one of the best spells of reverse-swing I've ever seen. It was awesome. We are talking serious, serious potential, huge talent and immense competitiveness."
Statistically then, there's little difference between them. At first class level at least, both comfortably pass the old test of having a batting average that easily outstrips their bowling average, and each could reasonably expect to hold a place in their county side on the strength on one skill alone. Both are still a long way shy of reaching that standard in international cricket. But then few players ever have. Bacher, whose book includes more complex calculations of what makes a good Test all-rounder, reckons there have only been 42 in the history of the game.
England then, are lucky to have two players who could be capable of qualifying, given time. The next Ashes may come too soon for either. All-rounders, who have so much to master, require an age to mature. Flintoff played Test cricket for five years before he reached his peak. But England have rarely had two such talented young contenders as Woakes and Stokes. It will be intriguing to see which of them gets the job.

Los Angeles: A newlywed US woman has been charged with murder for allegedly pushing her husband off a cliff barely a week after they married, according to court documents published on Tuesday.
Jordan Linn Graham appeared in court in the US state of Montana on Monday accused of second-degree murder over the death of her husband Cody in July in Glacier National Park, where they had gone hiking.
The 22-year-old initially told police her husband, 25, had gone missing after telling her he was going for a drive with a friend. But she subsequently admitted she had lied, and described how a cliff-top row turned to tragedy.
"Graham stated their argument intensified. At one point in time during their arguing, Graham turned and began to walk away. She stated Johnson grabbed her by the arm," said a police affidavit published the the Missoulian newspaper.
"After removing Johnson's hand from her arm, Graham stated she could have just walked away, but due to her anger, she pushed Johnson with both hands in the back and as a result, he fell face first off the cliff," it added.
Johnson's body was found on July 12 below The Loop, a well-known lookout area in the Montana park. An autopsy on July 19 concluded that the fatal injuries were consistent with a fall from a cliff.
The couple had married eight days before the July 7 alleged fatal encounter, the Missoulian reported.
A friend of the defendant identified as K.M., cited in the police affidavit, told detectives that Graham told her she was having second thoughts about having gotten married, and planned to talk to her husband about it.
Shortly before 9:00 pm on July 7 Graham texted her friend saying: "Oh well, I'm about to talk to him."
"I'll pray for you guys," K.M. replied.
Graham texted back: "But dead serious if u don't hear from me at all again tonight, something happened."

Moily to decide on RIL relinquishment proposal
A proposal to force Reliance Industries to give up 86 per cent of eastern offshore KG-D6 gas block, including 8 gas discoveries worth at least USD 8 billion, has been put up to Petroleum Minister M. Veerappa Moily.
The proposal based on recommendation of the Directorate General of Hydrocarbons (DGH) was put to Mr. Moily by his ministry a few days back, top officials said.
The Ministry, in the note put to Mr. Moily, concurred with DGH's reasoning that RIL had overshot the time allotted to it for developing the area and so should be as per contract be asked to relinquish most of the KG-D6 area.
The officials said Mr. Moily has to decide if leniency has to be shown to RIL since it has made several discoveries in the area and should be allowed to retain those areas for development.
Rejecting RIL's offer to relinquish 4,233 sq km of "low prospectively area" in the KG-DWN-98/3 or KG-D6 block, the DGH had stated that the company should contractually give up 6,601 sq km out of the total 7,645 sq km total area in the block.
If Mr. Moily decides to accept RIL's offer to enable the company to develop all of the 19 oil and gas discoveries made in the block so far, the proposal will have to go to the Cabinet as it would amount to making changes in signed production sharing contract, officials said.
DGH Director General R.N. Choubey had on April 15 written to Oil Secretary Vivek Rae that of the 19 oil and gas discoveries claimed by RIL, three finds have not been established as commercially viable in absence of test data and the company has not submitted any investment plans for another five.
According to DGH's own calculations, 6,601 sq km of contract area proposed for cessation has at least 1.15 trillion cubic feet of known recoverable gas reserves valued at USD 4.83 billion at current prices.
Of the 19 finds, RIL began crude oil production from MA field in September 2008. It started gas output from MA field and Dhirubhai-1 & 3, the largest of the 18 gas discoveries in the block, in April 2009.
Contractually, companies are required to relinquish 25 per cent of the area in an oil and gas block at the end of first phase of exploration that spans three years.
At the end of second phase, 50 per cent of the area is to be given up and by the third phase only such area is allowed to be retained where the company has made a discovery and is required for development and production of the same. The second and third phases are of two years duration each.
RIL and its partner Niko Resources of Canada were awarded the KG-D6 block in 2000. The three-year Phase-1 ended on June 7, 2003 while the 2-year Phase-II expired on June 7, 2005. The third phase ended on June 7, 2007.
Sources said DGH in 2006 agreed to RIL proposal of declaring the entire 7,645 sq km as discovery area, thereby allowing the company to retain the full area.
The decision was ratified by a committee headed by Additional Secretary in the ministry and by the Oil Minister thereafter.
But the decision was questioned by government auditor CAG as at the end of the third phase, only 79 per cent of the block area was covered by 3D seismic survey and yet the entire area was declared a discovery area.
CAG in its performance audit in 2011 had asked the ministry to review determination of entire contract area of KG-DWN-98/3 (KG-D6) as 'discovery area'.
KG-D6 output has dipped from 69.43 million standard cubic meters a day achieved in March 2010, to under 14 mmscmd this month.
